Health services closer to families
Getting guidance on available health care options is an important need for many families. Bringing these services closer to the community can make it easier to access information and care opportunities.
During the community health day held in collaboration with Sun River Health, attendees were able to access health screenings, blood pressure checks, and information to schedule medical appointments.
The activity offered a space to learn about these services and connect with a health care provider.

A collaboration centered on well-being
The collaboration between Casa Comunitaria de Recursos (CCR) and Sun River Health brought together CCR’s community work with the services of a health care provider.
While Sun River Health provided the health services during the event, CCR helped bring this opportunity to community members.
These partnerships are part of CCR’s work to connect families with organizations, information, and resources available in Dutchess County.

More than 100 people reached
The initiative reached more than 100 people, reflecting the scope of this community collaboration.
In addition to providing health services, the activity offered an opportunity to share resources and facilitate communication between residents and a health care provider.

Building connections for our community
Health and well-being are among the needs CCR seeks to address by connecting with trusted institutions and resources.
This health day with Sun River Health shows how collaboration between organizations brings important information and services closer to families.
For Casa Comunitaria de Recursos, maintaining these connections is part of its commitment to making guidance and community support opportunities more accessible.
